Fasting does not reliably increase testosterone. In fact, in lean, physically active young men, controlled studies consistently show that intermittent fasting lowers testosterone, not the other way around. The mechanism isn’t simply that cortisol rises and crowds out testosterone. The bigger driver appears to be suppressed signaling from the pituitary gland, specifically reduced luteinizing hormone (LH), which is the hormone that tells the testes to produce testosterone in the first place.
This is a case where popular fitness claims have gotten ahead of the evidence. Fasting has real, well-documented metabolic benefits, but boosting testosterone doesn’t appear to be one of them for most men, at least not directly.

A comprehensive review of human trials on intermittent fasting and reproductive hormones found that fasting reduced testosterone levels in lean, physically active young men, though it did not affect sex hormone binding globulin (SHBG) concentrations. Interestingly, the drop in testosterone did not appear to negatively affect muscle mass or muscular strength in these studies (NIH/PMC).
The length of the fast appears to matter. Research on longer fasting periods found that a 48-hour fast in men reduced LH, follicle-stimulating hormone (FSH), and testosterone concentrations, along with a decrease in LH pulse frequency, the rhythmic signaling pattern the brain uses to stimulate the testes. A separate study found that a 10-day total fast reduced both FSH and testosterone (NIH/PMC). This pattern, a drop in the upstream signaling hormones alongside testosterone itself, points toward the brain-to-testes communication pathway as the primary mechanism, rather than cortisol directly suppressing the testes.
The picture changes somewhat in men carrying excess body fat. A randomized controlled trial in obese men combining an intermittent fasting protocol with resistance training over eight weeks found that testosterone levels improved with the training, with no meaningful difference between the fasting group and the non-fasting control group. The fasting group also lost significantly more weight and fat than the control group (NIH/PMC). This suggests that in men who have excess fat to lose, fasting’s effect on testosterone may be more neutral, or even indirectly positive through fat loss, compared to its effect in already-lean men.
Why the “Just Cortisol” Explanation Falls Short
It’s tempting to explain fasting’s effect on testosterone with a simple story: fasting is stressful, stress raises cortisol, and cortisol suppresses testosterone. There’s some truth to this, since fasting can act as a physiological stressor and does influence the body’s stress response systems. But the research points to a more specific mechanism:
- Suppressed LH pulsing. The clearest finding across fasting studies is a reduction in luteinizing hormone pulse frequency, the signal from the brain that drives testosterone production at the testes. This is a distinct pathway from cortisol acting directly on the testes.
- Energy availability signaling. The reproductive system is highly sensitive to the body’s perceived energy status. Extended calorie restriction, even without unusual psychological stress, appears to signal to the brain that conditions aren’t favorable for reproduction, which can dial down the entire hormonal axis.
- Duration-dependent effects. Short daily fasting windows appear to have a smaller effect than extended fasts of 48 hours or longer, suggesting the mechanism is tied to sustained energy restriction rather than a brief spike in stress hormones.
What This Means in Practice
If you’re lean, active, and already have healthy testosterone levels, extended or aggressive fasting protocols aren’t a reliable way to raise testosterone, and may modestly lower it. The good news from the research is that this reduction hasn’t been shown to compromise muscle mass or strength in the studies conducted so far, so it’s not necessarily a reason to avoid fasting altogether if you enjoy it for other reasons.
If you’re overweight and fasting is helping you lose fat, the calculus is different. Since excess body fat independently lowers testosterone, weight loss achieved through fasting may offset or neutralize any direct hormonal cost, which is consistent with what the obese-male resistance training study found. As with most nutrition strategies, the right approach depends heavily on your starting point and goals, and it’s reasonable to monitor how you feel and consider blood work if you have concerns.
The Bottom Line
Fasting doesn’t reliably boost testosterone, and the “it’s just cortisol” explanation oversimplifies what’s actually happening. In lean, active men, fasting tends to modestly suppress testosterone by dampening the brain’s signaling to the testes, particularly during longer fasts.
